For anyone entering campus, the following form must be filled out and submitted online and a temperature check performed on campus prior to entering school.

Have you had symptoms of COVID-19* within the last 24 hours?
Has anyone in your household experienced symptoms of COVID19* or testing positive for COVID-19 in the past 24 hours?
Yes or No?
*Symptoms include fever, chills, cough, shortness of breath, difficulty breathing, fatigue, muscle/body aches, headache, new loss of taste or smell, sore throat, congestion/runny nose, nausea, vomiting, diarrhea.
Temperature Check Results:
1. Temperature reading less than 100.0° AND above questions are no: OK to permit onto campus.
2. If temperature reads 100.0° or greater, have person sit in a shaded area for five minutes before rechecking. If second temperature continues to read greater than 100.0°, send person home; for students, parent MUST be notified before student leaves campus.
